LEARNING OUTCOMES

The course is an application of procedures, actions and actions in Primary Health Care with the aim of providing students with the necessary knowledge to be able to understand and intervene in the needs of people with chronic diseases in the Community. The course material aims to expand the student’s knowledge-skills to prepare the person with a chronic illness or disability for a useful, productive and quality life in the social environment.

Upon successful completion of the course, the student will be able to:

  • knows the procedures for preparing the person with chronic diseases regarding self-care training during the post-clinical phase at home
  • assesses the needs of the person with a chronic illness or disability taking into account social and occupational changes
  • approach the disabled or chronically ill person by maintaining a positive image, a stable relationship with the family and active participation in the care or rehabilitation plan
  • evaluates the person by recognizing the starting points of secondary conditions (intense stress, depression)
  • plans in collaboration with the interdisciplinary team and the individual comprehensive rehabilitation care at home in liaison with the competent Primary Health Care services
  • educate the family of the chronically ill person on their health care plan
  • recognizes the needs of caregivers by providing health counseling and support
  • makes the local community aware of the care needs of these individuals and their integration into society

  • Stress, chronic illness and recovery
  • Community, health, rehabilitation – Interprofessional collaboration
  • Theoretical framework of home interventions
  • Home visit. Health care at home
  • Communication skills in the care of people with chronic diseases
  • Self care. Education and health counseling for the person with a chronic illness or disability
  • Training of caregivers in caring for individuals at home
  • Health Promotion and Health Education and rehabilitation interventions in the community
  • Care of people with chronic respiratory problems
  • Care of people with chronic circulatory system problems
  • Care of people with chronic cardiovascular system problems
  • Care of people with chronic problems of the musculoskeletal system 
  • Care of a person suffering from diabetes mellitus (adult-child)
  • Caring for people with dementia
